Diagnosis of venous insufficiency

CONSULTATION

During your consultation appointment, Dr. Ruz will take note of your medical history and will examine you. This will allow him to make a diagnosis and to explain the various treatment options and their risks. It is important to mention your expectations as they can be very different from one patient to another. Following the examination, Dr. Ruz will determine if the patient needs a Doppler, or if sclerotherapy treatments can start right away. He will estimate the number of treatments required. It is very important to understand that each patient responds differently to treatment and that it is impossible to accurately predict the total number of treatments at the first appointment. After a few sessions you will be able to judge if your body responds well to the treatments.

It is important to note that the first consultation is essential to an appropriate treatment plan. Often, patients may require the application of several therapeutic modalities to achieve adequate results.

PHYSICAL EXAMINATION

A physical examination by a doctor experienced in the treatment of venous diseases can quickly confirms whether the patient’s symptoms are related to venous insufficiency. There are multiple causes of lower limb pain such as muscle pain, joint pain, arterial insufficiency, etc.
The presence of telangiectasia, reticular veins and varicose veins does not necessarily mean that the pain comes from these, hence the importance of consulting a competent doctor.

To diagnose varicose veins, your doctor will do a physical exam, including looking at your legs while you’re standing to check for swelling. Your doctor may also ask you to describe any pain and aching in your legs. Accurate diagnosis is mandatory to define the anatomy and pathophysiology involved in the disease process.

ULTRASOUND – DOPPLER

In order to confirm a diagnosis of venous insufficiency of deep veins and to plan treatment, patients may need to have an ultrasound after their first visit. This must be done by a trained vascular sonographer or a vascular surgeon with additional expertise in vascular ultrasound.

Dr. Ruz uses high quality ultrasound transducer to allow visualization of the veins in high definition and to apply Doppler technology to measure venous reflux.

The ultrasound will allow to specify if the disease reaches only the superficial system or if the deep system and the perforators are affected. It will also measure the size of veins, quantify the degree of reflux, map varicose veins and plan their treatment. Venous ultrasound uses sound waves to produce images of the veins in the body. It is commonly used to search for blood clots, especially in the veins of the leg – a condition often referred to as deep vein thrombosis. Ultrasound does not use ionizing radiation and has no known harmful effects. Insightful information on how DU is performed during varicose veins work-up and the rationale of DU utilization for endovenous procedures are discussed.

The ultrasound is a painless procedure, performed while the patient is standing up and it may take up to 45 mins. At any time, the patient will be able to sit or lie down to take a break.

*In Quebec, the government does not reimburse the ultrasound for the accurate diagnosis of varicose diseases and ultrasound that you could have had in a hospital would not allow a doctor to be able to plan the treatment of your varicose veins.
